***If your child attends Forest Glen Middle School, we need your help! We need parents to volunteer with our girl mentoring group (Ladies of Distinction), a boy mentor group or serve as Parent Patrols.  PLEASE email me at levisarice@spsk12.net about your interests. We want our parents to be involved.

Welcome to our Wild Cat Guidance Page! My name is LeVisa Rice, and I will be serving Forest Glen Middle School for the 2018-2019 school year. In the Guidance department, I offer an open door policy where students and parents are able to contact the guidance counselor whenever they need to seek assistance. I am here at your service!

*During the summer, the Guidance Counselor is out until the end of August. All Guidance inquiries are addressed by the FGMS Main office.

-student shot records

-last student report card

-student schedule

-most recent IEP or 504 plan

-School withdrawal form
